Extreme heat has one particularly dangerous characteristic: its ability to gradually impair the body's capacity to recognize that something is wrong. The same physiological system that extreme heat overwhelms—the body's temperature-regulation system—is also responsible for allowing us to clearly perceive warning signs before the situation becomes an emergency.
How Temperature Regulation Works—and Why Extreme Heat Can Overwhelm It
The human body maintains an internal temperature of approximately 97°F to 99°F. When body temperature rises above that range, the body activates cooling mechanisms, including sweating and peripheral vasodilation, which redirects blood toward the surface of the skin.
Humidity is the most critical—and most underestimated—factor. When environmental humidity is very high, sweat cannot evaporate effectively, and the body's primary cooling mechanism stops working efficiently.
That is why a 95°F day with 80% humidity can be significantly more dangerous than a 100°F day with 20% humidity.
The Three Levels of Heat Illness: How to Recognize Each One
Heat cramps are the mildest form. They involve painful muscle spasms, primarily in the legs, arms, and abdomen, caused by the loss of electrolytes through sweating. The appropriate response is to stop the activity, move to a cool place, and hydrate with fluids containing electrolytes.
Heat exhaustion can cause heavy sweating, pale and clammy skin, weakness, dizziness, nausea, headache, and a rapid but weak pulse. It requires immediate action: move to a cool environment, elevate the legs, and apply cold compresses to the neck and armpits. Seek medical attention if symptoms do not improve within 15 to 20 minutes.
Heat stroke is the most serious emergency. Warning signs include a body temperature above 104°F, hot skin that may be dry, confusion, altered mental status, or loss of consciousness.

Who Is Most at Risk?
Adults over 65 have a reduced ability to regulate body temperature and may also take medications that interfere with sweating.
Children under four years old absorb heat from their environment more quickly than adults and may have a harder time communicating that they feel unwell.
People with chronic conditions such as diabetes, cardiovascular disease, or kidney disease can be more vulnerable because these conditions may interfere with the body's ability to regulate temperature.
Outdoor workers face prolonged exposure during the hottest parts of the day, while people who are not acclimated to high temperatures face a greater risk during their first two weeks of heat exposure.
The Rule That Removes the Doubt About When to Call 911
Prevention starts with proactive hydration. Thirst is a late sign of dehydration. On extremely hot days, drinking 16 to 20 ounces of water before going outside and approximately 6 to 8 ounces every 15 to 20 minutes during prolonged heat exposure can help maintain hydration.
And there is one rule that can eliminate uncertainty about when to call 911:
If someone is confused, unconscious, has hot skin, or does not respond to basic cooling measures after 15 minutes, treat it as a medical emergency and call 911 immediately.
Making that decision in time can be the difference between a frightening situation and a tragedy.
